Multiple Conditions.
One Coordinated Care.
Care that continues beyond visits.
CareMBrace is a connected care platform for people living with multiple chronic conditions.
It enables AI-assisted coordination across conditions and care partners — helping patients, providers, clinics and families stay connected through one coordinated care, making care more continuous, compassionate and easier to manage between visits.
Connected. Coordinated. Continuous. Compassionate.
Connected
Brings patients, providers, clinics, and families together.
Coordinated
Unifies care across conditions and care teams.
Continuous
Supports ongoing care between visits.
Compassionate
People-centered tools for real life, not just records.
Chronic disease is continuous. Care is still episodic.
The clinic visit creates a plan. The weeks between visits create the risk — and multimorbidity, not a single disease, is what most tools still fail to plan for.
Adherence drifts quietly
Medication schedules slip, walks get skipped, and nobody notices until the next appointment reconstructs it from memory.
Single-disease tools miss the whole patient
Most patients with multimorbidity are managing two or three conditions at once — not one. A diabetes app and a hypertension app that don't talk to each other still leave the gap open.
Families are left out of the loop
A caregiver finds out about a missed reading or a worsening symptom days later, if at all — by which point the moment to act has passed.
A coordinated loop, not a chatbot.
Five steps connect every check-in back to a clinician-approved rule — never to the AI's own judgment. The same loop runs across every condition a multimorbid patient has enrolled.
Enroll
The clinician sets the care plan and check-in rules for each of the patient's conditions — diabetes, hypertension and thyroid to start — as one coordinated plan, not three separate ones.
Check in
A WhatsApp bot collects vitals, medication adherence and symptoms on the schedule the clinician set — no app download required.
Structure
AI parses the free-text or voice reply into structured clinical fields. It extracts and classifies; it never generates a diagnosis or recommendation.
Apply rules
A deterministic, clinician-approved rule engine — not the AI — classifies each reading as Stable, Watch, Escalate or Emergency, and resolves which condition takes precedence if more than one flags at once.
Escalate
Only genuine exceptions route to a ranked clinician work queue. Stable readings log silently into the patient's unified record.
"AI reads and writes structured data. It never decides a clinical action on its own."
Built around the plan, not the app.
Every feature below exists to support one coordinated plan across a patient's full set of conditions — the core design decision behind CareMBrace.
One plan for multimorbidity
A single record spans every enrolled condition, with rules that resolve which one takes precedence when more than one flags at once.
WhatsApp-native check-ins
Patients reply where they already are. No new app, no login, no download.
Exception-based work queue
Clinician time scales with risk, not with message volume. Most check-ins never need a human.
Clinician-governed rules
Every threshold and every version is approved by a named clinician — never an engineering default.
Caregiver visibility
Family members get consented, scoped access to the plan — separate from the patient's own consent.
Full audit trail
Every alert cites the exact rule version and data that produced it — reconstructable months later.
A direct-to-patient line is also under evaluation for a future phase — gated on its own dedicated clinical-governance panel before it ever launches. It isn't available today, and we won't claim otherwise.
A published safety framework, not a black box.
Twelve safety principles govern exactly what the AI, the rule engine and the clinician are each allowed to decide — including how conflicting signals across multiple conditions get resolved.
A deterministic rule engine — not the AI — assigns severity. The AI only produces confidence-scored candidate data.
Low-confidence extractions and system failures route to a human review queue — never discarded, never defaulted to "normal."
A separate, high-recall check runs ahead of the main pipeline. A false alarm is an acceptable cost; a missed one is not.
Every rule, threshold and alert cites the exact version and data that produced it, with a permanent clinician approver.
When two conditions in a multimorbid patient's record flag at once, which one takes priority is a clinician-approved, versioned rule — never left to evaluation order.
Thresholds and the emergency glossary are only ever activated once a named, accountable clinician has reviewed them.
Built to the target, not waiting for the deadline.
India's DPDP Rules, 2025 (notified 14 Nov 2025) phase in substantive obligations over the following 18 months. We're designing to that bar from day one.
A standalone, itemized notice — never buried in terms and conditions. Withdrawal is a WhatsApp "STOP" reply, honored immediately.
Check-in data is used only for care coordination between visits — never for advertising, never sold to anyone.
Caregiver access is consented and scoped separately from the patient's own consent.
Data architecture is designed to map toward FHIR and ABDM concepts — a Phase 2+ roadmap commitment, not a certification we claim today.
We do not claim ABHA certification or ABDM-readiness we haven't earned yet. We'd rather be accurate about where we stand than overstate it.
Clinics and care teams managing multimorbid patients at volume.
Built for the clinics where chronic, overlapping conditions are the norm — not the exception.
Diabetes / internal-medicine clinics
Large chronic panels — often with diabetes, hypertension and thyroid together — currently relying on manual phone calls for follow-up, or on nothing at all.
Multi-doctor clinics & small chains
Need one consistent follow-up standard across doctors — not each one's personal WhatsApp habit.
Care-coordination teams
Nurses and coordinators who triage patient messages today, and need an exception queue, not more noise.
The buyer is the clinic. The user is the doctor and coordinator team — and the patient. This isn't a consumer app competing for a download.
Multimorbidity, by design
One patient record and one set of clinician-approved rules spanning every condition a patient has — not a separate tracker for each.
Safety you can read
A published, clinician-reviewable safety framework governs every decision boundary — not a black-box claim to "trust the AI."
Overlay, not overhaul
Works alongside the EMR or HMS a clinic already uses. No rip-and-replace, no new system to learn.
See it on your own follow-up workflow.
A 30-minute walkthrough on how check-ins, the rule engine and the clinician queue would fit your clinic's existing system — across every condition your panel manages.
